Social Prescribing Link Worker at Heaton Road Surgery
Introduction
I joined Heaton Road Surgery in 2020 as a Social Prescribing Link Worker, having previously worked at the children’s charity Barnado’s for 17 years as a Team Leader.
What I Do
I support patients and link them to community organisations and statutory agencies who work with them on issues such as benefits, housing, isolation, poverty and socialisation. I also run a Healthy Eating Group.
How I Support Patients
I meet with people to discuss the issues they’re facing, what they would like to be doing, and together we set an achievable plan. We’ll then catch up regularly to see how things are progressing. This means people feel supported through the process, and I can offer advice and encouragement along the way.
